About the mahi (role)

A Health Navigator will be primarily working on the Integrated Care Pathway for Musculoskeletal injuries (ICP-MSK). It is as a collaboration between TBI Health, Orthopaedic Specialists, and Physiotherapists to deliver an integrated rehabilitation programme for people with back, knee, and shoulder injuries. The main objective of ICP-MSK contract is to provide a high-quality client-centred service that is integrated, coordinated and measurable, improving access, experience, and outcomes for ACC clients. You will provide navigation support for clients ensuring they have a rehabilitation plan, they are making progress towards their goals and helping them navigate ACC supports to ensure their treatment, social, work and cultural needs are met. You will also play a key part of building and maintaining relationships with our partners ie surgeons, GP’s and other allied healthcare providers in the region.

There are several regions where we run the service and you be part of a wider national team of Health Navigators. In this role, you will be responsible for supporting the ICP-MSK for the Taranaki Region.

The successful candidate will assist with these key tasks:

Coordinate clients moving through the ICP-MSK Pathway.
Ensure an excellent client experience through their injury journey.
Ensure the client has access to the right ACC supports.
Champion cultural safety, equity and access for all kiritaki / clients.
Link in non-injury supports where identified.
Develop strong relationships between all rehab providers and stakeholders to deliver a high-quality interdisciplinary team service.
Be responsible for the contractual compliance and oversight of the ICP-MSK to ensure the success of the service.

About you

The applicant requires a minimum of 3-years Rehabilitation/ Health/ Case Management experience. A post-graduate qualification in rehabilitation/ occupational health would be an advantage to the applicant however not essential.
